Ordering “Home Care"?

Clients from the Baffin do not qualify for ADP or CCAC assistance.
 
IN NUNAVUT

The federal government and Nunavut have developed a home care system in the Baffin region. In each community, the level of service varies depending on the resources available.  If a patient needs home care, in Nunavut, please, call OHSNI’s case managers.   They will investigate what is presently available.  As well, they will let you know what paperwork is needed to process the request for service.

IN OTTAWA

The Government of Nunavut, in partnership with Health Canada, pays for limited home care services for a citizen of Nunavut who must stay in Ottawa for tertiary health care.  
 
OHSNI will accept the “CCAC" form as a requisition to initiate the process. As soon as the need for home care has been identified,  fax the “CCAC" form to the OHSNI case managers at 613-523-8163.   They will contact the ward staff to discuss prescriptions and equipment needs. 

OHSNI obtains approval from NU Health Insurance prior to ordering home care. Procedures like IV or IM medications, dressings, post-chemo assessments are accepted.  Personal care, like bathing or bed care, is not.

OHSNI will contract to the CareFor, nursing home care visits.  The “CCAC" form is their medical orders.  The case managers will so arrange for the delivery of medications and supplies.  

This process is time-consuming. Arrangements need to be started one to two days prior to discharge.  Arranging care for patients with more complex needs may take even longer.
