Ottawa Health Services Network Inc. is actively involved in discharge planning. Our patients live over 1000 kilometers from Ottawa and its medical services.  OHSNI’s staff works closely with southern and northern caregivers to ensure that the patient’s needs are met.

 

Remember the caregivers in the north are generalists. They need documentation to continue the care plan that was established while the patient was is the city. 

As the patients live in Nunavut, the system in place for Ontario residents does not apply.  OHSNI’s case manager is your best resource when discharging a patient from Nunavut.
