The Baffin-Ottawa Program invites any patient, or their escort, to contact our office for support or assistance. We are here to be of assistance to every Nunavummiut.

Baffin-Ottawa Program staff processes all medical referrals between Qikiqtani Regional Hospital and Ottawa. We inform Nunavut transportation office of the dates and times all appointments in Ottawa. Please contact the Out of Territory referral staff at Qikiqtani General Hospital if you have any special requests or need a date changed. They will contact Baffin-Ottawa Program Patient Support Coordinator to process these requests. Direct calls to Ottawa Specialists will lead in confusion, frustration and missed appointments.

Baffin-Ottawa Program Nurse Case Managers can help with issues such as locating medical equipment or organizing "nursing home care" for patient unable to return north due to a complex medical treatment. Please call them if you have any concerns and need advice or assistance.

For your medical records to be transferred from Ottawa to your northern caregiver, you may need to sign a Release of Information form. They are available at the Baffin-Ottawa Program office. You can drop in during office hours to sign one. (8: 00 AM to 5 PM Monday to Friday). If you are in the north and need access to these files, please, check with the staff at Qikiqtani General Hospital or at your local health center.
